        <title id="abstract-title-ec6b63f0a36c">Abstract</title>
        <p id="paragraph-730068453a8f"><bold id="s-15fe2cbe8ecf">Introduction</bold>: Head and neck squamous cell carcinoma (HNSCC) is the sixth most common cancer in the world. Abnormal methylation can be one reason for this cancer.  This study aimed to investigate the DNA promotor methylation status of cancer-associated genes (<italic id="e-82da52380327">ATM, APC, CDO1, RB1, TP53,</italic> and <italic id="e-afbe03e16fae">WIF1</italic>) in patients with HNSCC. <bold id="s-ba09c7ac7bcb">Methods</bold>: Bisulfite conversion and methylation-sensitive high-resolution melting were used to analyze the DNA methylation levels in normal and tumor tissues in 44 patients. <bold id="s-0074130f1347">Results</bold>: Significant differences in DNA methylation were observed between tumor and normal tissues for <italic id="e-90093f6f4626">CDO1</italic> and <italic id="e-d4bb4c34e392">WIF1</italic> genes in all subjects and subgroups (p &lt; 0.05). In the T<sub id="subscript-1">3</sub> subgroup, a significant correlation was found between CDO1 gene methylation and age in normal tissue. The same correlation was detected for the WIF1 gene methylation in tumor tissue samples in the subgroup with T<sub id="subscript-2">3</sub> and normal tissue samples in the subgroup with T<sub id="subscript-3">4</sub> (p &lt; 0.05). In all genes, no significant differences were found between the patient subgroups (T<sub id="subscript-4">2</sub>, T<sub id="subscript-5">3</sub>, T<sub id="subscript-6">4</sub> stage, primary/recurrent lesion, non-keratinizing/keratinizing SCC, age before/after 50, and smokers/non-smokers). <bold id="s-7b280f4f05ab">Conclusion</bold>: Changes in the expressions of <italic id="e-380a6eee8601">CDO1</italic> and <italic id="e-f52fbfd02cbd">WIF1 </italic>genes can affect the mechanisms of the occurrence and development of HNSCC. Methylation in the ATM, APC, RB1, and TP53 genes is not specific to HNSCC. </p>

      <title id="t-e5b81994a20b">Introduction</title>
      <p id="p-f0b0e91ed149">Epigenetic gene regulation changes (such as DNA methylation, histone modification, chromatin remodeling, and non-coding RNA) and DNA mutation are the basis of the molecular mechanisms of cancerogenesis and tumorigenesis. Therefore, studying epigenetic markers for diagnosis, prognosis, and cancer prevention is particularly interesting. Detection of DNA methylation levels has recently been used as biomarkers for early diagnosis and prognosis to identify target genes for drug therapy.</p>
      <p id="p-3d55b0ab5fd4">The hypomethylation of single CpG dinucleotides has been one of the first changes found in tumor cells<bold id="s-87f6d9d8ce75"><xref id="x-ac7373e3f0e0" rid="R222838530124141" ref-type="bibr">1</xref></bold>. A large part of the investigation has been carried out during DNA methylation in the promoter region of various genes<bold id="s-98f672aca3e7"><xref rid="R222838530124142" ref-type="bibr">2</xref>, <xref rid="R222838530124143" ref-type="bibr">3</xref></bold>. There are changes in the methylation levels of general and tissue-specific cell genes in the ontogenesis <italic id="emphasis-1">in </italic><italic id="emphasis-2">utero</italic> and post-natal periods. </p>
      <p id="p-e377a87b8534">The non-mutagenic etiology of cancer, such as disturbance of DNA methylation, is influenced by age, diet, transplacental, environmental, and occupational exposure. Moreover, its dual ability to be reversible and transgenerational demands a specific approach to interpreting results. Age-related DNA methylation is associated with different diseases<bold id="s-09007c133ce6"><xref id="x-a647f0b4b19c" rid="R222838530124144" ref-type="bibr">4</xref></bold> and sex hormone status in some cancer types<bold id="s-6dccb641d154"><xref id="x-173a5ce5be4f" rid="R222838530124145" ref-type="bibr">5</xref></bold>. There is a significant gap in understanding the association between the age-expected DNA methylation level and cancer<bold id="s-020a1d62f19a"><xref id="x-bf8c129536ba" rid="R222838530124146" ref-type="bibr">6</xref></bold>. In particular, no data exist regarding pre- and postmenopausal differences in DNA methylation levels and their effects on cancer development. </p>
      <p id="p-f5d58f134b90">The most intensive processes are in the genome of active proliferating cells, such as epithelial cells. Therefore, epithelial cells in various cancers are convenient models for studying epigenetic malformation. Head and neck squamous cell carcinoma (HNSCC) is an example of oncopathology with epithelial tissue damage.  </p>
      <p id="p-74ee07df3b3c">HNSCC corresponds to the majority of cases (~90%) of head and neck cancers (HNCs)<bold id="s-1fbd314494dd"><xref id="x-b1766f53ea68" rid="R222838530124147" ref-type="bibr">7</xref></bold>. HNCs are rated 6–7 in global prevalence among cancers, with 700,000 newly registered cases and 470,000 deaths a year<bold id="s-046f8006979b"><xref id="x-6d01525a0e70" rid="R222838530124148" ref-type="bibr">8</xref></bold>. The therapeutic options for HNCs include surgical treatment, radiotherapy, and chemotherapy. The management tactics should be personalized by a multidisciplinary board comprising an oncological surgeon, a chemotherapist, a radiotherapist, a psychologist, a rehabilitation specialist, and a dentist<bold id="s-935d1905260c"><xref rid="R222838530124149" ref-type="bibr">9</xref>, <xref rid="R222838530124150" ref-type="bibr">10</xref></bold>.</p>
      <p id="p-ff99156b548a">Despite the advanced diagnostics, cancer screening, and HPV vaccination programs, 60–70% of the cases are diagnosed as late as during stage III–IV, associated with low life expectancy and high risks of recurrence<bold id="s-d7fff628c698"><xref id="x-a0bb29b4f992" rid="R222838530124151" ref-type="bibr">11</xref></bold>. Therefore, finding new potential targets for therapy is crucial.</p>
      <p id="p-34ea7d7ee976">The main risk factor for HNSCC is smoking. In some publications, it has been shown that smokers have a 10-fold higher risk of developing the disease than non-smokers. Smoking, in combination with frequent alcohol consumption, increases the risk more than 35-fold<bold id="s-df66d822e9d7"><xref rid="R222838530124152" ref-type="bibr">12</xref>, <xref rid="R222838530124153" ref-type="bibr">13</xref>, <xref rid="R222838530124154" ref-type="bibr">14</xref></bold>. Other risk factors include ultraviolet and ionizing radiation, various toxic compounds, a weakened immune system, a diet low in vitamins A and B, age over 40, male, and some viral infections<bold id="s-303d9a01f3ad"><xref id="x-e9a98faabb4c" rid="R222838530124155" ref-type="bibr">15</xref></bold>. </p>
      <p id="p-8fae14516e3b">DNA methylation also varies due to several factors, including age and disease status. Silenced genes are often methylated, while active genes remain largely unmethylated<bold id="s-a9f249220caa"><xref rid="R222838530124156" ref-type="bibr">16</xref>, <xref rid="R222838530124157" ref-type="bibr">17</xref></bold>. DNA methylation is the most studied epigenetic mechanism in HNSCC. It allows us to observe changes in methylation patterns in a whole genome and individual genes that can be used to identify new disease biomarkers<bold id="s-ed689fe1a09f"><xref id="x-d34d15028e21" rid="R222838530124158" ref-type="bibr">18</xref></bold>. Various biomaterials can be used for studies, including liquid biopsy samples, which provide a noninvasive alternative for early cancer detection. In particular, Zhou C. studied 27 aberrantly methylated genes with altered expression and showed that FAM135B methylation is a favorable independent prognostic marker for overall survival in patients with HNSCC<bold id="s-005c23c85330"><xref id="x-40826465d77f" rid="R222838530124159" ref-type="bibr">19</xref></bold>. </p>
      <p id="p-4eb5b46c1a67">We previously investigated the DNA methylation status of some tumor-associated genes (<italic id="emphasis-3">CDO1, MEST, RASSF1A, RASSF2, RASSF5</italic>, and <italic id="emphasis-5">WIF1</italic>) in patients with HNSCC<bold id="s-7ec4b2d0a95f"><xref id="x-c6e0dbae7f29" rid="R222838530124160" ref-type="bibr">20</xref></bold>.  We observed significant differences in DNA methylation levels between tumor and normal tissues in the <italic id="emphasis-6">CDO1</italic> and <italic id="emphasis-7">WIF1</italic> genes in all patient groups and subgroups (larynx and other cancers, SCC keratinizing and non-keratinizing, primary and recurrent tumor, and smokers and non-smokers)<bold id="s-123e52acfab1"><xref id="x-b73a2cf2a683" rid="R222838530124162" ref-type="bibr">21</xref></bold>. The methylation level in the<italic id="emphasis-8"> CDO1</italic> gene in tumor tissue was significantly increased in the T<sub id="s-9b3d7c76d20c">4</sub> and T<sub id="s-e63c6789b90f">3</sub> stage subgroups over the T<sub id="s-ee106c5f0b00">2</sub> stage<bold id="s-39f5fe2e7212"><xref id="x-6b1cf2987552" rid="R222838530124164" ref-type="bibr">22</xref></bold>. </p>
      <p id="p-89b9eb7cc02f">For further research, we investigated another group of tumors and normal tissue from patients with HNSCC. We chose other cancer-associated genes: <italic id="emphasis-9">ATM, APC, RB1, </italic>and <italic id="emphasis-10">TP53</italic>. We also used these earlier studied genes (<italic id="emphasis-11">CDO1</italic> and <italic id="emphasis-12">WIF1</italic>), as they had shown notable results in other patients (<bold id="s-929e5a2ceafc"><xref id="x-c1f129e124f8" rid="tw-0d3332ba0df2" ref-type="table">Table 1</xref></bold>). The selected genes play a considerable role in the regulation of cell proliferation, differentiation, and apoptosis, the disruption of which can lead to oncopathology.</p>
      <p id="p-460a3bc74ebd">This study aimed to investigate the DNA promotor methylation status of cancer-associated genes (<italic id="emphasis-13">ATM, APC, CDO1, RB1, TP53, </italic>and<italic id="emphasis-14"> WIF1</italic>) in patients with HNSCC.</p>

      <title id="t-b23ecaed4c25">Methods</title>
      <sec>
        <title id="t-2b721842af17">Patient characteristics</title>
        <p id="p-b939a2ad0464">The study involved 44 patients (34 men and 10 women) with HNSCC treated at A. Tsyb Medical Radiology Center, Obninsk, and P.A. Herzen Cancer Research Institute, Moscow. The study was approved by the A. Tsyb MRC Ethics Committee (approval № 634-17.11.2021).</p>
        <p id="p-67b06fe24173">For the first step of therapy, all patients underwent surgery. The absence of other treatments before surgery was the main inclusion criterion.</p>
        <p id="p-36bff4c12ef9">The sampling time duration was 6 months. All patients were sampled before starting further therapies. The characteristics of the patients are listed in Table 2. In total, 37 patients were diagnosed with primary HNSCC, and tumor recurrence after therapy was observed in seven cases. Squamous cell carcinoma of the tongue was observed in nine cases, cancer of the oral cavity was observed in six, cancer of the floor of the mouth was observed in three, cancer of the maxillary sinus was observed in six, and larynx cancer was observed in 20 patients. The non-keratinizing tumors were observed in 28 patients, with the keratinizing type observed in 16 patients [21]. Of the 44 patients, 26 were smokers. The mean age of the men was 58 (30÷72), and that of the women was 58 (47÷81) years old. Distribution by age in all patients is shown in <bold id="s-193cc0b39a7c"><xref id="x-0ddf16825359" rid="f-6fed4f19f3b6" ref-type="fig">Figure 1</xref></bold>: four patients were 30–40 years of age (9%), eight patients were 41–50 years (18%), nine patients were 52–59 years (20%), 20 patients were 61–70 years (44%), and four patients were 71–81 (9%). The patients are classified according to the tumor, node, metastasis (TNM) system<bold id="s-9b3a99e3df00"><xref id="x-9f513d422514" rid="R222838530124164" ref-type="bibr">22</xref></bold>, in which T<sub id="s-792fb8d7486c">1</sub> - 2 (5%), T<sub id="s-c5d561d2624a">2</sub> - 7 (16%), T<sub id="s-3fd0025f513b">3</sub> - 20 (45%), and T<sub id="s-7689020728f7">4</sub> - 15 (34%). </p>
        <p id="p-62d871eb7311">The pre-operative contrast-enhanced CT images of three patients with various diagnoses are shown in <bold id="s-7f5fca4a3e2f"><xref id="x-dad9319a1555" rid="f-55eff9b149a8" ref-type="fig">Figure 2</xref></bold>.</p>
      </sec>
      <sec>
        <title id="t-705e8452f856">Sampling </title>
        <p id="p-ae8be31aa7c2">Tumor and normal tissue samples from each patient were obtained during surgery and stored at −20°C.</p>
      </sec>
      <sec>
        <title id="t-90555855012f">DNA extraction </title>
        <p id="p-3b8ef0fd4b79">DNA isolation from biomaterials was performed on microcolumns (K-SORB, № EX-514, Syntol, Russia) following the manufacturer's instructions.</p>
      </sec>
      <sec>
        <title id="t-4d085f47fe1a">DNA methylation analysis </title>
        <p id="p-a7ea2a058e4e">Bisulfite conversion was performed using the EZ DNA Methylation-Lightning Kit (ThermoFisher EpiJET Bisulfite Conversion Kit, K1461, ThermoFisher Scientific, Waltham, MA, USA) following the manufacturer’s instructions.</p>
        <p id="paragraph-12">Methylation of the promoter regions of the genes was performed using the methylation-sensitive high-resolution melting (MS-HRM) method using the CFX 96 Connect real-time system (BioRad, USA). </p>
        <p id="paragraph-13">The primers for the reaction were selected using Primer Blast software (Table 3). Ready-mix (PCR-Mix, M-428, Syntol, Russia) was used for two-step PCR. The program of amplification included 95°С — 5 min; (95°С — 15 s, 60°С — 30 s, 72°С — 45 s) × 30 cycles; (95°С — 15 s, 50°С — 30 s, 72°С — 45 s) × 25 cycles<bold id="s-5b871de4ec96"><xref id="x-279b2c7292f9" rid="R222838530124165" ref-type="bibr">23</xref></bold>. Furthermore, the intercalating dye EVA Green (Syntol, Russia) was added to the obtained products. Each sample was run in duplicate. The melting curve was constructed according to the following program: first stage - 95° - 30 s; second stage - 60° - 10 min; third stage — melting analysis in the range of 60–90° with a 0.2° step. MS-HRM was performed using Precision Melt Analysis Software, version 3 (BioRad, USA). A CFX96 amplifier (BioRad, USA) was used for PCR and MS-HRM. The methylation level was detected by fluorescence expressed in relative fluorescence units (RFU)<bold id="s-19fd8845d566"><xref id="x-0d7757592646" rid="R222838530124166" ref-type="bibr">24</xref></bold>. </p>
      </sec>
      <sec>
        <title id="t-1c257774b9f9">Statistical analysis </title>
        <p id="paragraph-15">Statistical analysis of the data was carried out using R language. The method used was a chi-squared test. P values less than 0.05 were considered statistically significant. </p>
      </sec>
    </sec>
    <sec>
      <title id="t-12329d06e266">Results</title>
      <p id="p-22d7e4a3eefb">The average methylation level results of the DNA promoter for all normal and tumor tissues of the patients and subgroups are shown in Table 4. Significant differences in DNA methylation levels between the tumor and normal tissues were found for<italic id="e-f388f5396cc1"> CDO1</italic> and <italic id="e-1a1845c30df1">WIF1</italic> genes in all patients (p &lt; 0.05). Significant differences were also observed for <italic id="e-b8dbd92e8309">CDO1</italic> and <italic id="e-a7ddcac27613">WIF1</italic> genes in the tumor and normal tissues in the different subgroups (p &lt; 0.05). The exception was the <italic id="e-f608b6849166">CDO1</italic> gene in a subgroup of patients with T<sub id="s-b6a5e46dc4c4">2</sub> and non-smokers. Regarding the <italic id="e-5df98849ad7a">WIF1</italic> gene, the exceptions were found in subgroups of patients with T<sub id="s-4dd20220d49b">2</sub> and T<sub id="s-8bd4512fe3fe">3</sub>, patients over 50, and non-smokers. The methylation levels of the DNA promoter of <italic id="e-32f9ea835022">ATM, APC, RB1, </italic>and <italic id="e-b35f43d19d28">TP53</italic> genes were insignificant. Furthermore, no significant differences were found between the subgroups (T<sub id="s-2f1b87212ba6">2</sub>, T<sub id="s-f5269a72f9f2">3</sub>, and T<sub id="s-14fb95a894fc">4</sub> stage, with primary and recurrent lesions, non-keratinizing and keratinizing SCC, before and after 50 years of age, and smokers and non-smokers) of the patients.</p>
      <p id="p-4707ece57491">Afterward, significant differences were observed in the methylation level between the tumor and normal tissues only for the <italic id="e-d8b72b2c8228">CDO1</italic> and <italic id="e-7723b31510af">WIF1</italic> genes. Using different TNM systems, we analyzed the relationship between these data and age in the patient samples (<bold id="s-66f22493d53f"><xref id="x-0d4edb57432f" rid="f-a7d727de44e4" ref-type="fig">Figure 3</xref></bold>). We observed a significant positive correlation in the methylation level in the <italic id="e-d8645e7190d1">CDO1</italic> gene with age in normal tissue from patients with T<sub id="subscript-7">3</sub>  (Age/Normal: y = −0.3102 + 0.0085*x; r = 0.66; p = 0.002; r<sup id="superscript-1">2</sup> = 0.43). The correlation between the methylation level in <italic id="e-c10938f8c931">CDO1</italic> with age in tumor tissue from patients with T<sub id="subscript-8">3</sub> was not significant but was different (Age/Tumor3: y = −0.0641 + 0.0086*x; r = 0.42; p = 0.07; r<sup id="superscript-2">2</sup> = 0.17). We observed a significant correlation between methylation level in the <italic id="e-ed34cc1cf5eb">WIF1</italic> gene and age in the tumor tissue samples from the subgroup with T<sub id="subscript-9">3</sub> (Age/Tumor3: y = −0.4654 + 0.013*x; r = 0.59; p = 0.006; r<sup id="superscript-3">2 </sup> = 0.35) and normal tissue samples in the subgroup with T<sub id="subscript-10">4</sub> (Age/Normal: y = −0.0002 + 0.0019*x; r = 0.52; p = 0.05; r<sup id="superscript-4">2</sup> = 0.27).</p>

      <title id="t-aa9c4ef2b146">Discussion</title>
      <p id="p-79ca4d8f9a44">Epigenetic disorders in tumor tissue cells in various types of oncopathology are as important as mutations, according to abundant literature data from the last decade. Early-stage epigenetic alterations can be identified and exploited to diagnose tumors early and predict cancer prognosis<bold id="s-8ccc22f7436b"><xref id="x-5b90e3563da8" rid="R222838530124167" ref-type="bibr">25</xref></bold>, and DNA methylation can be used as a molecular target for cancer treatment because it is reversible via pharmacological inhibition of DNA methyltransferase.</p>
      <p id="p-1196d09b4d95"><italic id="e-31d3780ca66d">(CDO-I)</italic> Cysteine dioxygenase type 1 protein-coding gene and tumor suppressor. <italic id="e-d204c2dba18d">CDO1</italic> expression may be silenced by promoter DNA methylation. <italic id="e-6effb06a4652">CDO1</italic> can augment the generation of reactive oxygen species to induce apoptosis. Many studies have shown increased promoter methylation and, consequently, suppression of the <italic id="e-c3b5b063d779">CDO1</italic> gene in various tumor cells<bold id="s-82aa55c5572b"><xref id="x-c5bde6dcaf89" rid="R222838530124168" ref-type="bibr">26</xref></bold>. In primary breast cancer patients and those with prostate cancer, the methylation level of the <italic id="e-b2e9670a33b5">CDO1</italic> gene's promoter region correlates strongly with tumor development. It can be utilized as a reliable prognostic indicator<bold id="s-cc490a4e4fb8"><xref id="x-492ce70ace6b" rid="R222838530124169" ref-type="bibr">27</xref></bold>. High levels of promoter <italic id="e-7bbe29571644">CDO1</italic> methylation have been observed in Barrett esophageal adenocarcinoma and prostate cancer patients<bold id="s-a78a91c4356c"><xref rid="R222838530124170" ref-type="bibr">28</xref>, <xref rid="R222838530124171" ref-type="bibr">29</xref></bold>. Our findings align with the evidence from the literature because the overall group of patients with HNSCC and subgroups had considerably higher <italic id="e-14314f209fc8">CDO1</italic> gene promoter methylation levels in tumor tissue than in normal tissue. Moreover, significant differences were found between smokers and non-smokers.</p>
      <p id="p-b56531ed475f"><italic id="e-29292def0e55">(WIF1) </italic>WNT inhibitory factor 1 is a protein-coding gene that is a tumor suppressor gene. These genes are epigenetically silenced in various cancers. The protein encoded by this gene functions to inhibit WNT proteins, which are extracellular signaling molecules that play a role in embryonic development. Wnt signal transduction dysregulation can contribute to head and neck cancer. Inhibition of Wnt signaling induces apoptosis and inhibits tumor growth in many cancer types<bold id="s-5cec83f56135"><xref id="x-9e08621754ad" rid="R222838530124172" ref-type="bibr">30</xref></bold>. Mutations in the genes encoding proteins in the Wnt (Wingless/Int-1) pathway are rare in HNSCC. Thus, this pathway is not considered significant for the pathogenesis of head and neck carcinomas<bold id="s-db41da1a4af7"><xref id="x-5ac4a1cb4cd2" rid="R222838530124173" ref-type="bibr">31</xref></bold>. </p>
      <p id="p-585946482dc2">Few studies exist on the methylation levels of the <italic id="e-cc63f25deda9">Wnt</italic> gene. Our data indicated no significant differences in normal and tumor tissues from patients in terms of the DNA methylation level of the <italic id="e-5a8264bef6f1">Wnt</italic> gene.</p>
      <p id="p-dbf62d7ef216">Methylation of the <italic id="e-f6e079c676b6">WIF</italic><italic id="e-47c3ed493b7c">1</italic> gene, one of the antagonists of this pathway, is often associated with the development of this pathology<bold id="s-560f4c5f542b"><xref id="x-db5d9cb21bcd" rid="R222838530124174" ref-type="bibr">32</xref></bold>. <italic id="e-981c9ec3ae32">WIF1</italic> has the highest methylation frequency in oral carcinomas. <italic id="e-7cff166a32f0">WIF1</italic> is methylated in 18% of patients with oral squamous cell carcinoma<bold id="s-dd2109eb6ee9"><xref id="x-56194e954cc0" rid="R222838530124175" ref-type="bibr">33</xref></bold> and 35% of patients with tongue carcinoma<bold id="s-e828f0ce1d29"><xref id="x-8a43622353fe" rid="R222838530124176" ref-type="bibr">34</xref></bold>, and it is significantly methylated in nasopharyngeal tumors<bold id="s-bde2f7b4cfc4"><xref id="x-30c574d23542" rid="R222838530124177" ref-type="bibr">35</xref></bold>. In addition, non-small cell lung cancer frequently exhibits hypermethylation of the <italic id="e-ee5cc3c87eb4">WIF1</italic> (Wnt inhibitory factor-1) promoter region, even though patient characteristics, such as age, sex, and smoking history, are unrelated to the methylation status<bold id="s-24bd731d038b"><xref id="x-8b4a2b278e8b" rid="R222838530124178" ref-type="bibr">36</xref></bold>. </p>
      <p id="p-72cb4eaf187d">Our study showed substantial variations in the average <italic id="emphasis-15">WIF1</italic> promoter methylation levels between tumor and normal tissues in the entire patient population and in the subgroups. There were also significant differences in this parameter in the tumor tissue in the subgroups of patients with keratinizing and non-keratinizing HNSCC. </p>
      <p id="p-c55c86eed600">It is well known that DNA methylation is associated with cancer and aging. Aberrant DNA methylation frequently occurs with aging and illnesses such as cancer. DNA methylation status can become disrupted according to age and a particular disease stage. For example, promoter-specific hypermethylation and concurrent gene silence are linked to various malignancies<bold id="s-4a415a440752"><xref id="x-3558290b0fb8" rid="R222838530124179" ref-type="bibr">37</xref></bold>. We found a significant correlation between methylation level for the <italic id="emphasis-16">CDO1</italic> and <italic id="emphasis-17">WIF</italic><italic id="emphasis-18">1</italic> genes in the normal and tumor tissues and age only in the subgroups with T<sub id="s-8c165216c3ca">3</sub> and T<sub id="s-de3f8fb98c82">4</sub>. These data indicate some dependence of the methylation level on the patient's age. The individual epigenetic features of the patient’s genome likely play an essential role in the results obtained. Thus, the study should be repeated with a larger sample size, and/or patients should be stratified according to other criteria.</p>
      <p id="p-e59d1633bda4"><italic id="e-96babb4ee848">Adenomatous polyposis coli </italic>(<italic id="emphasis-19">APC</italic>) is a tumor suppressor gene that inhibits cell proliferation through the Wnt/β-catenin signaling pathway. <italic id="emphasis-20">APC</italic> interacts with DNA repair proteins, DNA replication proteins, tubulin, and other components. It is not expressed in some types of cancer, in particular prostate, breast, and colorectal cancer. Lack of expression is associated with reduced survival in cancer patients. Moreover, <italic id="emphasis-21">APC</italic> regulates the response to chemotherapy in cancer cells<bold id="s-04c306e873db"><xref id="x-67db616e4208" rid="R222838530124180" ref-type="bibr">38</xref></bold>. The Wnt/β-catenin signaling pathway is activated by <italic id="emphasis-22">APC</italic> mutations in colorectal, endometrial, and prostate malignancies<bold id="s-df7407e4f724"><xref id="x-bea21ff687e3" rid="R222838530124181" ref-type="bibr">39</xref></bold>. Hypermethylation of this gene has been detected in oral squamous cell carcinoma samples in some studies but not in patients with head and neck cancers. Additionally, there are no differences in the methylation of the <italic id="emphasis-23">APC</italic> gene between HNSCC patients and healthy individuals. The <italic id="emphasis-24">APC</italic> promoter was methylated in 7% of DNA samples taken from the plasma of a population free of cancer. It was unrelated to several other putative risk factors, such as age, tobacco and alcohol use, family history of cancer, diet, and nutrition. Other research failed to find a connection between clinical traits and outcomes, such as survival and aberrant methylation<bold id="s-6007a76aa79d"><xref rid="R222838530124182" ref-type="bibr">40</xref>, <xref rid="R222838530124183" ref-type="bibr">41</xref></bold>. Our results showed no significant differences in DNA methylation for the <italic id="emphasis-25">APC</italic> gene between tumor and normal tissues. </p>
      <p id="p-e7406d569349">The ataxia–telangiectasia mutation (<italic id="emphasis-26">ATM)</italic> gene product is a well-known tumor suppressor essential for maintaining genomic stability. The mutated form of the <italic id="emphasis-27">ATM</italic> gene is involved in cell cycle control, apoptosis, oxidative stress, and telomere maintenance, and its role as a risk factor for cancer development is well established. In addition, several studies have been conducted to evaluate the risk of <italic id="emphasis-28">ATM</italic> gene mutations for breast cancer. In addition, several studies have been conducted to evaluate the screening of <italic id="emphasis-29">ATM</italic> gene mutations for the occurrence of breast cancer<bold id="s-8328473be92b"><xref rid="R222838530124184" ref-type="bibr">42</xref>, <xref rid="R222838530124185" ref-type="bibr">43</xref></bold>. In HNSCC, the <italic id="emphasis-30">ATM</italic> gene promoter is a target for abnormal hypermethylation. Reduced <italic id="emphasis-31">ATM</italic> function through epigenetic silencing is a likely mechanism contributing to HNSCC and other tumor types, given its significant role in maintaining genome integrity and the causal role that genome instability plays in cancer onset and progression. It has been found that hypermethylation of the <italic id="emphasis-32">ATM</italic> promoter is significantly correlated with a decrease in the average survival rate of patients. Furthermore, this link is unrelated to other possible predictive elements, such as tumor size, lymph node condition, clinical stage, and history of tobacco and alcohol use. Thus, these data indicate that hypermethylation of the <italic id="emphasis-33">ATM</italic> promoter can be used as a prognostic factor in HNSCC<bold id="s-e49a0dd7c5e9"><xref id="x-9632041dd996" rid="R222838530124186" ref-type="bibr">44</xref></bold>. However, our data revealed no significant differences in DNA methylation levels for the<italic id="emphasis-34"> ATM</italic> gene in<italic id="emphasis-35"> </italic>the tumor and normal tissues of patients.</p>
      <p id="p-0bec484f524c">Functional properties caused by tumor protein p53 (<italic id="emphasis-36">TP53</italic>) gene mutations are involved in cancer development and progression<bold id="s-cf51a095c5b0"><xref rid="R222838530124187" ref-type="bibr">45</xref>, <xref rid="R222838530124188" ref-type="bibr">46</xref></bold>. Somatic mutation is one of the most prevalent changes in the <italic id="emphasis-37">TP53</italic> gene in human tumors. Moreover, the underlying cause of Li-Fraumeni syndrome, which predisposes patients to various early-onset cancers, is germline mutation. <italic id="emphasis-38">TP53</italic> gene alterations are also possible prognostic and predictive indicators and prospective drug targets<bold id="s-2e82f64b826a"><xref id="x-293f197f5b3c" rid="R222838530124189" ref-type="bibr">47</xref></bold>. According to a thorough integrative genomic study, most somatic genomic changes in HNSCC are mutations in the <italic id="emphasis-39">TP53</italic> gene. Invasion, metastasis, genomic instability, and cancer cell proliferation are all facilitated by <italic id="emphasis-40">TP53</italic> mutations, which can result in a loss of wild-type <italic id="e-474e360c36bb">p53 </italic>activity or an increase in those activities. Interestingly, aggressiveness and worse survival following surgical treatment of HNSCC are related to disruptive <italic id="emphasis-41">TP53</italic> mutations in tumor DNA<bold id="s-dc78efccdfea"><xref id="x-ee5bfc6acb4d" rid="R222838530124190" ref-type="bibr">48</xref></bold>. Harris C.C. proposed that genetic analysis of the <italic id="e-085d5b7aed0a">p53</italic> gene in exfoliated cells detected in either body fluids or tissue biopsies might identify people at elevated cancer risk because mutations in the <italic id="e-72100c3ef8fb">p53</italic> gene can arise in precancerous lesions in the lung, breast, esophagus, and colon<bold id="s-2a9a2a311064"><xref id="x-4437dc4652ad" rid="R222838530124191" ref-type="bibr">49</xref></bold>. Hypermethylation of the promoter region of <italic id="emphasis-42">TP53</italic> is often associated with a decrease in its activity and can even lead to its silence and, thus, to the loss of its function. This, in turn, contributes to malignant transformation<bold id="s-25e845e90e53"><xref id="x-f1a2d2befdb5" rid="R222838530124192" ref-type="bibr">50</xref></bold>. Unfortunately, our research showed no significant differences in DNA methylation for the<italic id="emphasis-43"> TP53 </italic>gene between<italic id="emphasis-44"> </italic>tumor and normal tissues in the patients.</p>
      <p id="p-84d2ebb4a0be">The tumor suppressor retinoblastoma gene (<italic id="emphasis-45">RB1</italic>) is essential for controlling the cell cycle by increasing G1/S arrest and limiting growth by blocking the E2F transcription factor. Even aneuploidy can result from <italic id="emphasis-46">RB1</italic> loss, dramatically increasing the chance of developing cancer. The <italic id="emphasis-47">RB1 </italic>gene is part of a large gene family that includes RBL1 and RBL2, each encoding the structurally related proteins pRb, p107, and p130<bold id="s-c0352abe7cc3"><xref id="x-841f93a42c4d" rid="R222838530124193" ref-type="bibr">51</xref></bold>. One frameshift and seven non-synonymous missense mutations were discovered in <italic id="emphasis-48">RB1</italic> (pocket domain and spacer region) sequencing analysis. In HNC tumor samples, an <italic id="emphasis-49">RB1</italic> promoter methylation study showed that 16% of its cytosines (3% in CpG) were methylated<bold id="s-8c13c2560782"><xref id="x-c5cf26ee8ead" rid="R222838530124194" ref-type="bibr">52</xref></bold>. Sabir M. and colleagues indicated that genetic and epigenetic <italic id="emphasis-50">RB1</italic> changes might contribute to the pathogenesis of HNSCC in the Pakistani population<bold id="s-25c054497e82"><xref id="x-4701b4545aef" rid="R222838530124195" ref-type="bibr">53</xref></bold>. However, our study showed no significant differences in DNA methylation for<italic id="emphasis-51"> </italic>the<italic id="emphasis-52"> RB1</italic> gene between<italic id="emphasis-53"> </italic>tumor and normal tissues in our examined patients.</p>
      <p id="p-b193155f5ce5">Even though in our patients with HNSCC, in the tumor-associated genes <italic id="emphasis-54">APC</italic>, <italic id="emphasis-55">ATM</italic>, <italic id="emphasis-56">TP53</italic>, and <italic id="emphasis-57">RB1</italic>, no significant changes were observed in the methylation level between normal and tumor tissue, these genes are not specific for epigenetic analysis.</p>
      <p id="p-c712cedda2b1">We also did not find significant differences in any genes (T<sub id="s-7d2db446e4b3">2</sub>, T<sub id="s-27201a7ae3a3">3</sub>, and T<sub id="s-aed5f2da6c8b">4</sub> stages, with primary and recurrent lesion, non-keratinizing and keratinizing SCC, age before and after 50 years, and smokers, and non-smokers) of the patients. This may be due to the small number of patients examined. </p>

      <title id="t-bbd98129805a">Conclusions </title>
      <p id="p-f45720805a87">Thus, we can conclude that changes in the expressions of the <italic id="emphasis-58">CDO1</italic> and <italic id="emphasis-59">WIF1</italic> genes can affect the occurrence and development of HNSCC and can be considered prognostic and diagnostic markers for this pathology. Future application of molecular–genetic and epigenetic studies on HNSCC can serve as the basis for target therapy and contribute to personalized medicine.</p>
